OE4.A1.1: Digital transformation of healthcare into primary and community care
The Ministry of Health, together with the CC.AA. e INGESA works in a coordinated collaborative way in projects of digital transformation of health care in primary and community care with focus on 7 areas:
Specifically, in the Health sector, the Mission was defined as “Creation of an intelligent system of data analysis that allows to anticipate and act early and ultra quickly in the face of the main physical and psychological diseases in 2050, in the context of the decentralization of the health system and aging.”

OE4.A1.2: Personalized Digital Attention Plan and UNIQUE Project
Also in a coordinated and collaborative way, the Personalized Digital Attention Plan is being developed, in which 5 type projects aimed at providing tools that facilitate Personalized Digital Attention in the SNS have been identified. An analysis of the available tools and an identification of shortcomings has been carried out, and work is being done on the implementation of the identified developments.
As for the UNIQUE (sjdhospitalbarcelona.org) project, it is a network project, composed of 30 hospitals (at least one per autonomous community), which will have digital systems for the care of pediatric patients with minority diseases and support for their families, thanks to a network working model of health professionals specializing in these pathologies.
